It looks like the set screw backed out of the coupler, and now I can’t seem to get the holes from the coupler to align with the holes in the shaft that comes out of the transmission. I can get them lined up within about 2mm, but not enough to get the set screw in...... any ideas?

try pulling the shaft out alittle bit ir line it up with a 1.5mm allen wrench and kinda wiggle the wrench will adding downward presure to the wrench it should be rather easy
