Potentially Stupid Battery Question

So I have a genesis 450mah 3s lipo battery (four of them actually) but number four does not like to charge. I plug it into my charger and in 5 minutes my charger alerts me to the battery being fully charged, however, when I plug the battery into my rig all it can do is turn the servo and when checked with a multimeter it reads dead. Any insight? I feel like I could be missing something.

No swell to the encasement ?

do you balance those each time you charge them ?

what charger - method ?
