Default 12mm-17mm adapter idea?

Does anyone know where I can find a 12mm-17mm adapter that adds little to no offset? Thanks

Edit: I drew this up in paint and was wondering if there was anything like this or how much it would cost to get one made. or would this even work

I've got a set that is exactly like that, got em a few years ago, now I just need to figure out where I got them...

interesting, just had someone come into our shop looking for something like that, if you were local, I would have thought it was you!

you might ask around and see if anyone local can machine you something, I would avoid a commercial machine shop, they always rape you with the price.
