Default rear axle shaft coming out of locker

I have disassembled teh rear diff a few times now in search of this problem and Iam stumped. My right rear axle shaft keeps coming out of the locker. With the heel bolted on, I can not get it back in, If I take the wheel off I get the shaft to pop right back in. I have grabbed both wheels and pulled on them to try to feel any play and I feel none. I put the truck on the ground and go about five feet and I only have 3 wheel drive. Happens all the time now and it sucks.

Are you running straight axles or the dogbone/axle shaft setup?
If it's the latter then make sure you have the o-ring (or a small piece of fuel tubing) inside the axle cup.
If you don't the axle will back out.
